1. Service-Connected Disability Retirement
Staff Recommendation:
1. David L. Phelps is permanently incapacitated for the performance of duties and a disability retirement should be granted.
2. David L. Phelps’ disability is service-connected based on the claimed injury(ies)/disease as listed in Exhibit A; therefore, grant a service-connected disability benefit.
3. Staff shall initiate disability benefit payments with an effective date of September 20, 2025, the day after the last day of regular compensation.
