1. Service-Connected Disability Retirement
Staff Recommendation:
1. Alex Cundieff is permanently incapacitated for the performance of duties and a disability retirement should be granted.
2. Alex Cundieff’s disability is non-service connected based on the claimed injury(ies)/disease as listed in Exhibit A; therefore, grant a non-service connected disability benefit.
3. Staff shall initiate disability benefit payments with an effective date of the day after the last day of regular compensation as determined by staff.
