Requested Benefit:
1. Service-Connected Disability Retirement
Staff Recommendation:
1. Roxanne L. Logan is permanently incapacitated for the performance of duties and a disability retirement be granted.
2. Roxanne L. Logan’s disability is service-connected based on the claimed injury(ies)/disease as listed in Exhibit A; therefore, grant a service-connected disability benefit.
3. Staff shall initiate disability benefit payments with an effective date of March 17, 2018 the day after the last day of regular compensation provided the benefit is requested in writing.
