Requested Benefit:
1. Service-Connected Disability Retirement
Staff Recommendation:
1. John D. Emmens is permanently incapacitated for the performance of duties and a disability retirement be granted.
2. John D. Emmens’ disability is service-connected based on the claimed injury(ies)/disease as listed in Exhibit A; therefore, grant a service-connected disability benefit.
3. Staff shall initiate disability benefit payments with an effective date of October 27, 2018, payable to Christine Emmens, the surviving spouse of Mr. Emmens, the day after the last day of regular compensation provided the benefit is requested in writing.
