Requested Benefit:
1. Service-Connected Disability Retirement
Staff Recommendation:
1. Heather Hubbard is permanently incapacitated for the performance of duties and a disability retirement be granted.
2. Heather Hubbard’s disability is service-connected based on the claimed injury(ies)/disease as listed in Exhibit A; therefore, grant a service-connected disability benefit.
3. Staff shall initiate disability benefit payments with an effective date of May 10, 2018, the date of application.
