Review and determine the disability retirement application of Dana A. Carter - County of San Bernardino - Probation.

Requested Benefit:

1.                     Service-Connected Disability Retirement with Supplemental Retirement Allowance.

 

Staff Recommendation:

1.                     Dana A. Carter is permanently incapacitated for the performance of duties and a disability retirement be granted.

 

2.                     Dana A. Carter’s disability is service-connected based on the claimed injury(ies)/disease as listed in Exhibit A; therefore, grant a service-connected disability benefit.

 

3.                     Dana A. Carter is capable of gainful employment; therefore, deny the supplemental disability retirement allowance.

 

4.                     Staff shall initiate disability benefit payments with an effective date of May 12, 2018, the day after the last day of regular compensation.
